Person Centered Housing Options

Homeless outreach covers pcho staff out on the streets of rochester, ny, and the surrounding monroe county. The outreach team works to connect and build relationships with homeless individuals and ultimately get them placed into agendas which will assist them in obtaining permanent housing and other assistance through coordinated entry.

Care management includes pcho's health homes program; a medicaid-funded program that provides care management services to eligible medicaid enrollees with their care needs so they can stay as healthy as they can be.
