Through a person centered planning process, comprehensive supports coordination, best practice treatments, and rehabilitative approaches, individuals will learn, grow, and enjoy a variety of new experiences, relationships, and activity. By providing needed supports, individuals have the ability to create independent and meaningful everyday lives.
