Through care management and trust services, we support individuals with a mental illness, intellectual disability &/ or physical challenges to live happier, healthier, more meaningful lives.

Special needs trust administration - we serve as a fiduciary of special needs trusts so that people with a disability can preserve their benefits and set aside assets to supplement those benefits. As a corporate fiduciary, we manage all assets in the trust for the sole benefit of the beneficiary. We assist the person or family with understanding how to set up a special needs trust, deepening their understanding of options and possibilities. If named as the fiduciary, we work with the person and support system to understand how the trust operates and establish goals. We determine a budget, develop an investment strategy and establish a shared understanding of how trust assets intend to be used. The trust may hold investments, liquid assets or real property. Our fiduciary services focus on assisting the beneficiary to stabilize after a traumatic life circumstance, gain long term stability and achieve ongoing health, happiness and meaning in their lives. We engage financial and legal advisors as well as community contractors and professionals to preserve the assets in the trust for the benefit of the person with a disability. Plan of pa is able to serve as the corporate fiduciary of a first party or third party trust as well as a first or third party pooled trust.
Representative payee - plan of pa serves as an organizationalrepresentative payee for adults with a behavioral health diagnosisand/or intellectual or physical disabilities receiving ssi & ssd benefits from the social securityadministration. As representative payee, plan of pa arranges foran individual, interest-bearing checking account, assures promptpayment of bills for basic food, shelter and medical needs andthen helps develop a realistic, individualized budget for anyremaining funds. All of the individuals we serve under this program must have a funded trust or active care management.
