Positive Impact Health Centers Inc

Client services: provided 26,096 units of service to 4,567 patients. Services were provided for medical case management, non-medical case management, housing, food vouchers, meal/grocery delivery, translation, medical transportation and patient navigator activities. These services were funded through ryan white funding through hrsa directly or indirectly through fulton county and the ga dept of hhs, as well as samhsa directly and by howa funding, through the city of atlanta.

Prevention services: administered 5,500 hiv tests and identified 189 new hiv-positive cases; re-linked previously hiv-positive persons out of care back into hiv medical care; conducted std screenings; vaccinated patients against hepatitis a/b; and started individuals on prep. In addition, the organization provided one-on-one one-hour risk counseling sessions and over 191,209 free condoms were distributed to high-risk communities in metro atlanta through bars, sex clubs and free community condom dispensers. Prevention services were funded by the cdc, samhsa, the ga dept of hhs and fulton county.
