To provide unified supportive infrastructure that facilitates access to community based primary, behavioral and other health care services to every community in south carolina.

Scphca operates a health center voucher program, partnering with health centers and other healthcare providers across the state of south carolina to provide eligible farm workers and their families with access to comprehensive medical, dental, behavioral health, vision, pharmacy, and enabling services. Per 2024 uds reporting, the health center program provided 9,991 clinical encounters to 1,974 patients, exceeding the patient target and improved in nine (9) uds clinical outcome measures for the farmworker patients we serve. Developed a referral partnership with a local non-profit that supplies menstrual education and essential supplies and a group of ophthalmologists who provide eye surgeries and other advanced ophthalmologic care/services free of charge to those in need.

Scphca is the health center controlled network grantee for the state of south carolina. Through the hccn program, nineteen participating health centers receive support for emerging health information technology (hit), quality improvement, data analytics, and patient engagement. Using technology and these strategic objectives, the hccn helps to identify clinical quality measures that need to be addressed for target patient populations early in the year enabling proactive outreach to those patients most in need. The hccn further applies technology to reduce provider burden and increase patient engagement and satisfaction, thereby preparing health centers for participation in value-based care delivery models while building resilience for the future. During the grant period august 1, 2022 to july 31, 2025, the hccn grant saved participating health centers more than $1m for data analytics and reporting infrastructure and ai-enabled health quality improvement models through shared service and strategic purchasing agreements. The hccn partnered with two (2) south carolina managed care organizations (mcos) that covered implementation and ongoing subscription costs for participating health centers' access to a payer integration module to: reconcile differences between payer-attributed members and uds patient panels; support hedis and value-based care performance with up-to-date insights; improve collaboration across the care continuum to improve care coordination for high-risk patients through shared data utilization; and, streamlining reporting by aligning payer and provider data across population health metrics while also closing the loop for value-based contracts. Building on the work accomplished during scphca's designation as the hccn for south carolina, the hccn will work with twenty health centers in the state to address the objectives in the next iteration of this program includes data management and analytics, interoperability and data sharing, uds modernization, value-based care readiness, artificial intelligence and emerging technologies, and cybersecurity.
