A. To apply for federally qualified health center (fqhc) or fqhc look- alikestatus pursuant to section 330 of the public health service act, which health center will provide preventive and primary health care services, related enabling and ancillary services, and other critical community and social services including, but not limited to, health education, community outreach, environmental health services, nutritional services, oral health services, and behavioral services, regardless of a patients insurance status or ability to pay as required under united states health resources and services administration guidelines. The mission and vision of the health center shall be as follows: b. To serve as the community board of the health center in satisfaction of the community based governance requirements of section 330 of the public health service act, as amended; c.
