Health Access Point* The static site (UMOJA HAP Clinic: 5000 3rd Street) opened in Feb.* We added Glide as a subcontract partner to deliver services at Pop up sites and 5000 3rd Street. (SIT, HEPC, HIV testing, and Mental Health services)* We signed MOUs with The Drop- in Center, Mercy Housing, FRH Consulting, and The Phenix Project to become Satellite Site for monthly HAP services.* A soft lunch and health fair celebrating the opening of the 5000-3rd st. site.* A kickoff celebration and health fair in Visitation Valley(Sunnydale) Mercy Housing.* The lunch of the UMOJA HAP CAB (Community Advisory Board).* Hired full staff for EMOJA HAP. . HAP Program Manager . HAP Program Coordinator . HAP Data Analysis . HAP Test Coordinator* Providing administrative, navigation, mental wellness, youth groups, and other community centered activities at 5000 3rd Street site.
Client Services - Rafiki Case Management Services - Provided medical case management hours and case management group hours to HIV support groups and residents of Brandy Moore House, an 11-bed HIV/AIDS residential transitional housing facility owned and operated by Rafiki Coalition For Health and Wellness. Per a revised contract, Brandy Moore House no longer has to account for Case Management services or Group hours, just our Transitional Housing services.
