Comprehensive assessment - assists individuals with completing the developmental disabilities administation (dda) application process and provide referral for community resources based upon assessment of needs. The program processed 28 applications in the current fiscal year.
Waiting list coordination services - assists individuals who are on the developmental disabilities administration's (dda) waiting list for services. Case managers connect individuals with resources in their communities in order to stabilize crises and to promote independence and community participation. The program served 211 individuals during the current fiscal year.
