SOH WILL PURCHASE PERSONAL HYGIENE AND BABY ITEMS, COLD WEATHER AND HEALTH ESSENTIALS, SHELF-STABLE FOOD AND BEVERAGES, AND FRESH PRODUCE FOR STREET OUTREACH CARE PACKAGES. SOH'S STREET OUTREACH TEAM WILL DISTRIBUTE CARE PACKAGES ON A MONTHLY BASIS TO 20 INDIVIDUALS EXPERIENCING HOMELESSNESS IN EACH OF THE FOUR COUNTIES. SOH WOULD UTILIZE THESE DISTRIBUTIONS AS OPPORTUNITIES TO ASSESS NEEDS AND ENROLL INDIVIDUALS IN ITS CASE MANAGEMENT AND RAPID REHOUSING SERVICES. CASE MANAGERS WILL ASSIST WITH DISTRIBUTING CARE PACKAGES TO INDIVIDUALS WHO ENROLL IN SOH'S SERVICES BUT ARE NOT YET PERMANENTLY HOUSED. THROUGHOUT THIS PROJECT PERIOD, SOH WILL TRACK COMMUNITY ENGAGEMENTS, HOUSING ASSISTANCE PROVIDED, AND REDUCTION IN HOUSING WAIT LIST TIMES.
