Home visiting connecting nj universal hv south jersey is home to a wealth of health and social services. The difficulty is finding these programs and figuring out if you are eligible or a good fit for the service. Connecting nj eliminates this challenge and does the research for you. With its knowledgeable staff and state-of-the- art system, connecting nj can quickly provide quality referrals that meet an individual's or family's specific needs. While cooperative's focus is maternal and child health, connecting nj has information on a wide range of resources. Connecting nj counselors work closely with you to identify specific concerns and locate the services that sddress those needs. This service is free, confidential and caring.

Nurse family partnership/healthy families: nurse-family partnership is a free, evidence-based community health program that pairs at-risk, first-time birthing individuals with highly-skilled home-visiting nurses. Nurses visit clients homes during client pregnancies to ensure healthy pregnancies. After the birth of the child, visits continue for the first two years of the child's life, with nurses serving as a critical conduit, connecting families to healthcare services and educational resources on preventive health practices and parenting skills, so new parents are equipped new to give their child the best possible start in life.
