Mission

Axis health system (ahs) is the designated community mental health center, and a licensed substance use provider and a hrsa community health centers (fqhc) providing integrated primary care including oral health care. Ahs offers a wide variety and range of behavioral health and integrated primary care and oral health services to assist people in managing their health and health outcomes in southwest colorado. Essential behavioral health services include: screenings, individual, group and family therapeutic services, community resource coordination, outreach, short-term residential crisis stabilization, psychiatry, 24/7 crisis assessment services, vocational services, substance use disorder services, and dual diagnosis services for patients with cooccurring mental health and substance use disorders. Primary care integration includes behavioral and oral health across all ages using population based screening including screening for social determinants of health, depression and anxiety.

Integrated primary care - comprehensive primary care is available at ahs's integrated clinics. Behavioral health services and oral health services are part of the comprehensive care continuum at these community health center clinics. A total of 29,761 integrated service hours were provided to patients in fiscal year 2024.

Acute treatment unit - a residential program providing short-term psychiatric care and stabilization. It is the only psychiatric residential resource available in the entire southwest region of colorado. The atu provided 1,248 bed days to 226 residents of the region in fiscal year 2024.
