Care coordination unit care coordinators, under an illinois department on aging program, provide comprehensive in-home assessments and resource referrals to assist elderly, low income adults with getting the services they need to maintain a safe level of independence while living in their own chosen place of residence as long as possible. 10,900 persons were served during the year.

Adult protective services case workers investigate reports of abuse, neglect, and exploitation among the senior and disabled adult population. 1,513 persons were served during the year.
