In 2024, St Hope Foundation continued its mission to provide high-quality, comprehensive healthcare to all patients, delivering a total of 33,573 visits across 10,115 patients. Medical services accounted for 22,420 visits (5,661 patients), Behavioral Health 4,683 visits (1,510 patients), Nutrition 1,157 visits (1,080 patients), and Vision 4,156 visits (1,864 patients). The Medical Program achieved significant progress in improving patient outcomes. Eighty-five percent of patients living with HIV reach viral load suppression (<200), reflecting strong adherence and care coordination. Overall clinical quality improved by 34%, demonstrating continued excellence in service delivery. Chronic disease management also strengthened, with more diabetic patients maintaining A1C levels below 9, indicating better disease control. Additionally, expanded participation with a broader range of insurance carriers diversified the patient base and improved access to care. Through these efforts, St. Hope Foundation advanced its commitment of accessible, integrated, and patient-centered healthcare, resulting in measurable improvements in community health outcomes.
2024 Dental Program Overview: The program served over 6,000 patients with more than 20,000 visits. The most common procedures performed were extractions and cleanings, followed closely by fillings. Other services such as dentures (4%), crowns (3%), and root canals made up a smaller portion of care. Key Insight: Patients averaged approximately 3 visits per year, reflecting strong engagement with dental services.
