We empower youth and young adults who experience a developmental disability in leading a meaningful and inclusive life by providing high quality and person centered support and services to the persaon and thier family.

Programs

Supported Employment - Summit Future Foundation provides customized employment training services to individuals 16+. Participants will learn vital job readiness skills. Additionally, our supported employment programs assist individuals in seeking employment, obtaining employment and provide ongoing job coaching support to facilitate successful employment. During 2024, 9 individuals were Assisted in career exploration and discovery, 8 Supported in obtaining new employment, 17 Maintaining employment)

Community Engagement Services - designed to meet the unique social and emotional needs of the individuals we serve and promote growth in interpersonal skills through participating in community-based leisure activities. In 2024, 65 individuals participated in our various social and community engagement groups.

Watch Me Thrive - designed to support transition aged youth between the ages of 14-21 to develop employment readiness skills needed to enter the workforce after aging out of school-based services. In addition, Watch Me Thrive supports families in preparing for their childs future by providing comprehensive training and technical assistance. During 2024, 18 teens were supported in gaining work readiness skills and 60 families were guided in the process of creating an actionable transition plan for their child.
