Mission
Organization: sunshine terrace foundation is in our 77th year of service to our community. As a 501(c)(3) charitable non-profit health care provider as a non-profit foundation. We were formed to carry out a public purpose. We are honored to have 5 appointed trustee emeriti. We are governed by a board of directors with varied backgrounds. Our board members include successful educators, business men and women who volunteer from our community. They provide us with valuable business insght and strategic direction for our future. Through the collaborating efforts of a very supportive community network, our campus of care has reached literally thousands of individuals both inside out outside of our state who have needed health care assistance. Our vision is to bring more to life.
Programs

Terrace grove assisted living is a level two assisted living center licensed for 72 beds and 55 apartments. We are the only assisted living facility in cache valley that provides 24 hour nursing assistance. All of our staff play an important role in providing the highest quality of care at our facility. Residents of terrace grove have access to our award winning recreational therapy and music therapy. Terrace grove offers chef prepared and dietician approved meals and snacks. Residents have a home-like and safe environment with assistance with daily living activities.
Sunshine home health care and sunshine hospice is a licensed full service home health & hospice agency offering both skilled nursing and speech, occupational, and physical therapy. Our home health and hospice are both state-certified. We belong to the utah association of home care, utah hospice and palliative care organization, and the national association of home care & hospice. Sunshine home health & sunshine hospice provides education on health, medications and personal care to clients and loved ones in the home. Nurses deliver iv infusions, wound care, and diabetic care and acute and post-surgical care. Therapy is also provided to increase health, safety and independence in the home and community. In addition to these home based services, we have sunshine cares, a licensed personal care agency offering non-skilled and respite services. We service clients with non-skilled needs such as personal care, bathing & grooming, shopping and errands, homemaking and respite care.
The worx outpatient therapy & fitness, formally known as aquaworx physical therapy & fitness, is a premier community out-patient physical, occupational, speech, water therapy clinic, and fitness gym. Specializing in aquatic therapy, prehab (rehabilitation prior to a surgery to allow for quicker recovery), veritgo, dry needling, balance therapy, neurological treatments, and age forty and finer fitness training. Our state-of-the-art hydroworx pool and our licensed therapy staff help our patients heal from injuries and improve quality of life as they regain their abilities. We offer an exercise specialist in our fitness gym and classes. We have the only certified rock steady boxing course in northern utah. This course helps clients who suffer from parkinson, manage the disease process with dignity. We are proud to serve members of the silver sneakers program.
