Home Care - Taconic provides support services that allows individuals to live safely in their homes.

Respite program provides temporary, short-term relief for families and care providers which enables them to arrange for their vacations, emergency coverage in the event of family or provider illness or death, or for a break from constant, intensive participant care and supervision. During the year 2024, 231,569 units of service were provided.

HCBS Group Day Habilitation Service helps individuals with acquisition, improvement of self-help,socialization and adaptive skills including communication, travel and other areas in adult education This program is offered to people regardless of where they live. During the year 2024, 15,222 units of services were provided.

Other programs include other services for individuals with disabilities.

Supportive IRA provides practice in independent living under variable amounts of oversight delivered in accordance with the individuals needs for supervision. Supportive IRA also includes assistance with acquisition, retention or improvements of self-help skills related to activities of daily living, such as personalgrooming and cleanliness, bed-making and household chores, eating and the preparation of food, and the social and adaptive skills necessary to enable the individual to reside in a non-institutional setting. During the year 2024, 48 units of service were provided.

Individualized Support Services include rent subsidies and housing transition stipends paid on behalf of people with disabilities through a direct contract with OPWDD. During the year 2024, 252 units of service were provided.
