Acquired brain injury residential supports-this program transitions individuals currently living in nursing or rehab facilities into a 24 hour care management and community based supports program to individuals with acquired brain injuries. Our altr program also provides residential supports thru a "turning 22" program which facilitates home placement for young adults with autism who have turned 22 and are no longer eligible for child based autism services.
Adult foster care - provides services to massachusetts with developmental disabilities who need assistance with activities in daily living in order for them to safely live at home. Case managers and nurses for this service operate out of the attleboro locations.
