TLC Health & Wellness was founded with the mission to provide high-quality, compassionate, and comprehensive services to promote healthy living, long-term wellness, stronger communities. The agency provides housing and other supportive services to vulnerable populations.
Programs

The HUD Permanent Supportive Housing (PSH) Program grant provides housing and supportive services to persons 18 years and older who have medically documented disabling health conditions and are experiencing chronic homelessness.
The HUD Youth Homelessness Demonstration Program (YHDP) Supportive Services Only (SSO) Program grant, administered through the Coalition for the Homeless, provides coordinated entry assessments, referrals, and housing navigation services to youth and young adults ages 18 - 24 who are experiencing homelessness.
TLC Health & Wellness' Rapid ReHousing (RRH) program provides a short-term, one year, housing intervention to persons 18 years and older who are experiencing homelessness. Services include rental and utility assistance, case management, counseling, assistance with access to mainstream benefits, workforce development services, and access to other needed services.
Program income supports other critical services and necessities for clients.
