Community residential - supervised living residential services in four community group homes.

Day support - our center for learning provides a meaningful day program that promotes each participant's cognitive, physical, social and emotional well-being and growth.

Home & community based services - the center trains care providers to provide care in the homes of parents of children and adults who have developmental disabilies. Therapy services - the center utilizes an interdisciplinary approach, involving team members from different disciplines working collaboratively to perform assessments, diagnose and develop intervention plans. Tailored care management - tlc's tailored care management provides a dedicated care manager to coordinate all aspects of a person's health care and supports - medical, behavioral, long-term, and community services. Our care managers and care management team work together with an individual, their family and/or guardians, as well as their doctors, service providers, therapists, and others to coordiate a plan of care that is right for the person.
