Case management -community-based services - cohesive and coordinated systems of outreach and case management services facilitate living, advocacy and linkage to mental health and medical services.the behavioral health care coordination program delivers consumer-directed, recovery oriented care coordination and services to increase the individual's capacity to manage their health and wellness, live a self-directed life and reach their full potential.mobile outreach offers temporary case management and mental health treatment services while working on linking individuals to a long-term treatment provider of their choice. Adult protective services (aps) provides state mandated services to individuals suffering from mental and/or physical impairments. These individuals are unable to manage their own resources, carry out the activities of daily living or protect themselves from abuse, neglect, exploitation or other hazardous situations and have no one available and willing to assist them. Services are designed to prevent or eliminate risk factors by strengthening the individual's capacity to function and to self direct.
