Tri-valley, inc. Is an organization that is set up to assist eligible older residents with a range of services that will enhance their quality of life and allow them to live as independently as possible in dignity and respect.

Programs

Adult family care and personal care attendants - provides family living as an alternative to institutional care. The program matches participants with caregivers who provide 24-hour support in their homes or allows consumers to select, train and employ their own caregivers with support from social workers and nursing staff.

Senior care options - combines health service fee with social support services by coordinating care and specialized geriatric support services, along with respite care for families and caregivers.

Expenses: $4.7M

Nutrition and support - includes home-delivered meals to individuals who are unable to leave their homes without difficulty; community dining centers which create an opportunity for individuals 60 years and older an opportunity for socializing while enjoying a nutritious lunch; and emergency food pantry services.

Protective services - investigates and resolves allegations of elder abuse including physical, emotional, sexual, financial exploitation, neglect or self neglect.

Other programs - include: care transitions which help older and disabled adults transition from the hospital or rehab setting to home; congegrate housing which is a low-cost shared living experience in a home-like environment that is ideal for elders and younger disabled people who are looking for socialization and independent living; elder mental health outreach which increases behavioral health support in underserved communities; and money management which matches trained, insured and supervised volunteers with elders who need help balancing the checking account, setting up a budget, writing checks and sorting mail. Other services are provided to individuals through several different programs which are operated in conjunction with other agencies; programs which are not directly state supported efforts; and, programs which are designed to meet specific community/ individual needs.
