Trust For Health Care Benefits Plan For Employees Of Eatel Corp Inc

The plan provides health benefits (medical, hospital, surgical, major medical and limited vision) to full time employees of the sponsor who meet eligibility requirements and to their beneficiaries and covered dependents. Retired employees are entitled to similar health benefits provided they have retired from the sponsor in accordance with the sponsor's retirement policies and pay the stipulated contribution required by the plan.
