Covering kids & families of united health services (ckf-uhs) ckf is a coalition of local partners with one goal: total health coverage. Located in north central indiana, ckf-uhs is responsible for five indiana counties: elkhart, st. Joseph, marshall, starke and pulaski. Coalition partners are working collaboratively to reach and insure residents in hoosier healthwise, hip, medicaid and marketplace plans. Through this program we provide indiana state licensed and certified navigators who help consumers with questions and enrollment into the state and federal insurances available. Using a navigator increases the chances of those we serve being accepted into the program and provides for peace of mind during the application process. With our resources through this program, we often have extra information to help those se serve to make better informed decisions. Our navigators meet in person at local hospitals, health fairs, community events, and at our offices in granger, indiana to name a few of our many outreach efforts. We also provide virtual and telephone appointments and use an online scheduler to make the process easy and accessible to all. Additionally, ckf-uhs has navigators who speak english & spanish. In 2024 ckf of uhs: - processed over 650 applications for healthcare coverage - successfully enrolled and had accepted 610 of those applications - achieved and above average 93% approval rate on total applications - held or attended outreach events connecting with over 3,000 people

Breast health programs: in coordination with local hospitals and healthcare providers including 48 offices, 15 mammogram facilities, and 17 labs and radiologists the breast health programs facilitate community, state and federal funds to provide free mammography and diagnostic services for medically underserved or uninsured men and women in the community. Referrals and navigation are available for men and women diagnosed with breast cancer. Breast cancer is not just a woman's disease. Men do get breast cancer, though it is rare. We provide indiana state licensed and certified bilingual navigators to guide through the screening, diagnostic and diagnosis process. For nearly 28 years united health services has been a part of the breast & cervical cancer program (bccp). Due to our tremendous success with this program, we have grown to serving 27 counties. Through this idoh ongoing grant we coordinate the breast care coverage in these counties for women ages 30 and older who are uninsured and at or below 200% of poverty for screenings. Insured women 200% of poverty and below may be eligible for bccp to be their secondary coverage. This provides coverage for pap/hpv, clinical breast exams, screening mammograms, and diagnostic services based on age & clinical criteria. In 2024 breast health programs provided and/or coordinated over 2,300 area women with services related to their breast or cervical health.

The diabetes resource center of unitd health services works one on one with those in need of help gaining access to healthcare, especially their diabetic medication and supplies. Our goal is to relieve immediate crisis and then assist our clients into long term, stable solutions as we teach, provide services and support individuals living with diabetes and their families so that they can attain a healthy lifestyle. The drc can assist with the immediate need of insulin for qualified people that are 250% at or below poverty guidelines, lives in indiana, exhausted other options as well as pregnant and children at 350% or below poverty guidelines. With help from physicians & pharmacies across the region, the drc facilitates patient assistance programs & discount card programs for medications. Often this is the only way some individuals gain access to necessary medications. This allows local diabetics to get their medications directly from the pharmaceutical company, bringing in thousands of dollars' worth of medication each year. Each company has its own rules and eligibility standards, and our office works directly with anyone who need a referral or assistance. Our staff is up to date on new glucose meters and testing information. We train individuals who are newly testing or switching devices/models to assure accurate results. Often, we supply start up kits for the newly diagnosed. The drc staff is aware of indiana and federal laws regarding diabetics and are current on medicare and medicaid supply coverage. Any drc staff member is required to be an insurance navigator so they can mitigate the unnecessary use of community resources when people are eligible for coverage. In 2024m drc provided over 150,000 worth of medication assistance to 65 individuals. Suicide prevention center (spc) our mission is to prevent suicide through education, training and community-wide awareness and to offer compassionate grief support, resources to those who have lost a loved one to suicide. We aim to reduce the stigma associated with suicide and to achieve zero suicides in indiana. In working to accomplish our mission we have several areas of focus that include and are not limited to: -suicide survivor support group (sssg) -- for those who are coping with the loss of someone to suicide this group meets the last tuesday of the month from 6:30 pm to 7:30 pm at our united health services office in granger and provides for a virtual option. This is an opportunity to connect with others who have lost a loved one to suicide and a safe place to share experiences and emotions in a caring, warm and compassionate environment with people who understand the pain they are feeling. - suicide prevention training including: oliving works oqpr and asist (training available for a small fee to groups or individuals) oyellow ribbon campaign- to middle and high schools in 2024,we reached over 1,800 area students through its yellow ribbon teen suicide prevention program through 11 schools.
