For hospice care of vna, 92% of our families we served in 2024 give us the highest "top box" score in overall satisfaction with our agency. Our compassionate hospice teams enhance the final weeks, days and hours of our patients' lives. In 2024 vna's hospice team delivered world-class care to 352 patients in kansas city and the surrounding 11 county area. Our patients averaged 74.06 days on our hospice service in 2024 equates to an average daily census of 47. Additionally, our hospice professionals also provide invaluable end of life education to families and loved ones, and we continue providing care through our bereavement services for 13 months following the death of a loved one. At vna hospice we provide our services to patients of all ages and background.

The community outreach program has a focus on the 65 and over population in the kansas city metropolitan area as well as the surrounding missouri counties of cass, clay, jackson, platte and the kansas counties of johnson and wyandotte. The community outreach program completed a total of 3,848 patient encounters at 46 locations. Our low-income older adults served by this program were provided 5,415 screenings for blood sugar, cholesterol, and blood pressure. Our participants are forever grateful for vna providing a healthier outlook on their future. The demographic makeup of the community outreach program participants for 2024: - 94% age 60+ - 74% female - 65% caucasian - 20% african.
