The organization provides hospice care and support to terminally-ill patients and their loved ones living in vigo and surrounding counties in indiana. This is done through a team of medical directors and attending physicians, skilled nurses, medical social workers, home health aides, spiritual counselors, bereavement professionals and hospice trained volunteers. Over 22,268 days of service were provided to 662 patients during the 2024 year. Medicaid and medicare services accounted for approximately 95% of this volume. Charity care is available to patients who are unable to pay. Patients are identified based on financial information obtained from the patient and/or referral source and reviewed by the organization's team of directors and caregivers.

The organization also provides in-patient hospice care and support for patients that meet the medicare general-in-patient (gip) criteria and can no longer be cared for in their homes. This support is provided to terminally-ill patients and their loved ones living in vigo and surrounding counties in indiana. This is done through a team of medical directors and attending physicians, skilled nurses, medical social workers, home health aides, spiritual counselors, bereavement professionals and hospice trained volunteers. Over 1,004 days of service were provided to 339 patients during the 2024 year. Medicaid and medicare services accounted for approximately 88% of this volume. Charity care is available to patients who are unable to pay. Patients are identified based on financial information obtained from the patient and/or referral source and reviewed by the organization's team of directors and caregivers.community hospice agencies are contracted to use our in-patient facility. 49 days of service were provided to 14 of these patients.
