Vital Access Care Foundation

The community outreach, education & engagement program delivers outreach, education, and navigation on health and social determinants, including insurance, housing, chronic and infectious disease, and hate incident response. In 2024, vacf reached 279,964 individuals through media footprints, engaged 17,260 community members directly, and conducted 462 health classes, community trainings, and webinars. The program also provided a total of 40,546 referrals and linkages, including food, financial aid, mental health support, translation, legal aid, transportation, and health insurance enrollment. This program also provides food assistance to 9,880 households and distributed assistance to 20,657 individuals. In addition, vacf hosted and appeared on 55 radio shows, 35 tv/newspaper features, and conducted 92 community outreach events. All services were provided free of charge.

The equity coalition and community building program convenes stakeholders across sectors to address health disparities, housing, and workforce needs in the underserved aanhpi community through collective efforts and equity lens. The program provides trainings to improve the community capacity on different aspects, such as community resources, data inequity, quality improvement, and advocacy, to elevate the voices and representation of the aanhpi residents. In 2024, vacf facilitated various community listening sessions, focus groups, and convenings. The apitf direct care network is a project initiated by the apitf with the goal to increase capacity of the direct care workforce to provide culturally responsive care and services to aanhpi families living in southern california. The project aims to address the lack of direct care services and providers in the community. In 2024, through the project 211 aanhpi caregivers were trained and 340 classes were provided in various asian, native hawaiian, and pacific islander languages.
