Mission
Wcmschf's flagship program, project access of wake county (pa), has been providing access to donated specialty and diagnostic care for eligible uninsured residents of wake and franklin counties with household incomes below 250% of the federal poverty level for 21 years. Pa also navigates uninsured patients without a primary care home to affordable or free care at one of 12 partnering safety-net clinics. Pa staff also engage with program enrollees to assure that patients complete connections while helping build awareness and ability to self-manage the healthcare landscape.

Rightcare's community health navigators have provided one-on-one support to over 500 uninsured residents of wake county in 2024. These clients came to rightcare without insurance or a primary care home. The navigators assisted each individual to get connected become an established patient at an affordable - in most cases free - quality healthcare through expanded patient navigation services.
A community health worker case management framework is used to provide support and guidance to clients while aiding them in developing health literacy to manage the healthcare landscape successfully on their own.
