Willamette vital health offers adult foster care for those unable to care for themselves and for whom care at home is not a realistic or safe option. The edward f. Tokarski home for our hospice patients who need a place to call home in their final months of life. Individualized care is provided for up to 5 residents who are also enrolled as patients of willamette vital health. Eligibility depends upon specific care needs and space availability.
