Programs
3 programs
The healthy transitions program improves access to treatment and support services for youth and young adults, ages 16-25, who have a serious emotional disturbance or a serious mental illness. This will be accomplished by increasing awareness, screening and detection, outreach and engagement, referrals to treatment, coordination of care and evidence-informed treatment.
The ohiorise (resilience through integrated systems and excellence) system of care is a state program for youth with complex behavioral health and multi-system needs and wingspan is named the care management entity (cme) for lorain and medina counties. As a cme wingspan will provide eligible families with high-fidelity wraparound services that link them with mental and behavioral health providers, community-based services, and formal and informal networks of support in their communities. Wingspan will connect with these families through involvment with the juvenile justice system, addiction and mental health services, child protective services, school resources and other touchpoints.
The wuliger wellness center is an integrated, outpatient primary care facility that offers high-quality, affordable services in a uniquely designed environment. The program addresses barriers to access and offers specialized accomodations for individuals with autism and other developmental delays. Some of the services offered include preventative dental services and treatment for substance use disorder and behavioral health issues.
