Program 1
$22.0MPreventive health services (phs):sea mar's multi-functional phs team covers three major branches of service. Health education includes programs that provide patients with nutritional counseling and guidance, weight control, family planning, smoking cessation and chronic disease management support, and maternity support services (mss), which supports pregnant women and new mothers to ensure healthy pregnancy and infancy outcomes. In fye 2024, sea mar's mss, nutrition, and health education programs served 21,000 patients with 25,000 visits. Sea mar's women, infants, and children (wic) program is the largest private wic provider in the state of washington, serving 33,000 clients with approximately 178,000 encounters in fy 2024. The program provides food vouchers alongside health, breastfeeding, and nutrition education for pregnant women, new mothers, and young children.
Program 2
$13.3MChemical dependency inpatient treatment centers:sea mar goes beyond the outpatient treatment model to provide longer-term inpatient chemical dependency treatment for low-income, uninsured, or under-insured youth and adults in four facilities offering 100 beds throughout its service area. These treatment centers offer bilingual, culturally appropriate services to people with few other options for such treatment. In fy 2024, sea mar's inpatient treatment centers provided 31,167 patient days to approximately 1160 individuals with safe detox, evidence-based treatment, and guidance during re-entry, including transition to sea mar's outpatient behavioral health services as needed. Nearly all of these patients were living below the 200% federal poverty level.
Program 3
$5.1MCannon house assisted living facility:cannon house is located in the diverse central district neighborhood of seattle and provides safe and affordable assisted living services for seniors and others in need of residential care. The facility houses around 68 residents and focuses on helping residents maintain independence and a high quality of life. Sea mar provided 24,090 patient days to cannon house residents in fy 2024. Nearly all cannon house residents are medicaid/medicare recipients living below the 200% federal poverty level.
Program 4
$2.6MManaged care:the managed care department has made sea mar a leader in the state of washington for providing health insurance enrollment information and assistance to community members in need. In fy 2024, sea mar's managed care outreach and enrollment team served approximately 46,667 unduplicated individuals with information about insurance options and enrollment in insurance plans. Services provided are in person and over the phone, in english, spanish, and other languages as needed to serve the client. Patients who become medicaid beneficiaries and enter into managed care services with sea mar receive monitoring and outreach to ensure they are overcoming any barriers to accessing the services they need, and successfully engaging in treatment and follow-up to reach the positive health outcomes established between the patient and their provider. There are over 144,467 patients assigned to sea mar in the managed care services program.

$3.2MBeacon hill i & ii housing:in 2022, using public funds from the city of seattle and the state of washington, sea mar purchased a newly constructed 42-unit studio apartment building in the beacon hill neighborhood of seattle. One apartment is set aside for the office and another apartment is set aside for a live-in property manager. The apartments are income restricted and half of the apartments can serve households no more than 50% ami and the other half no more than 30% ami. However, most of the tenants have zero income because these apartments target the homeless population and provide housing specifically for women who were homeless at the time of application. Most of these applicants have experienced chronic homelessness throughout much of their lives. Residents have access to on-site case management services that can assist them in accessing healthcare, counseling, and financial assistance, as well as education, job training, and employment opportunities. In 2024, using public funds from the city of seattle and the state of washington, sea mar purchased another newly constructed apartment building in the beacon hill neighborhood of seattle. This building has 39 apartments which are exclusively studios and there is one apartment set aside for a live-in property manager. These apartments are income restricted and half of the apartments can serve households no more than 50% ami and the other half no more than 30% ami. However, most of the tenants have zero income because these apartments target the homeless population and provide housing specifically for men, women, and small families who were homeless at the time of application. Residents have access to on-site case management services that can assist them in accessing healthcare, counseling, and financial assistance, as well as education, job training, and employment opportunities.
Program 6
$4.8MSea mar properties washington i llc:across nine different properties located in seven counties on both sides of the cascade mountains in washington state, sea mar affordable housing provides clean, safe, and affordable housing to low-income families, individuals, seniors, and disabled persons living within rural communities. These nine properties consist of 285 apartment homes. Two of the properties are set aside for senior and disabled populations. The other seven properties are set aside for families and individuals. Of the 285 homes, 244 of them have project-based rental assistance which enables our tenants to not have to pay more than 40% of their income on the rent. This subsidy assistance is a major benefit for our tenants and provides a measure of stability and security for operations, as well. Each property is proudly owned and operated by sea mar community health centers.

$41.9MOther services:sea mar meets its mission by providing comprehensive services with a specialization in serving the latino community through a wide variety of programs beyond the core healthcare services. These programs focus on addressing the social determinants of health by providing various social supports that give individuals and families the support and connections they need to maintain well-being. These programs include affordable housing, nutrition, and services outreach for latino senior citizens, cancer screening programs, community events, citizenship classes, childcare, civic engagement, education, youth educational achievement and leadership programs, spanish-language radio programming, a boxing club for at-risk youth, home care and more. Through these programs, clients achieve independence and the skills necessary to succeed in their lives, which translates to success in achieving positive health outcomes over the long term.
