Pichc (perinatal infant community health collaborative)(5 year grant: july1, 2022-june 30, 2027- funded by nysdoh)primary focus: chrf's pichc program is an in-person home-visiting program that works together with communities to improve perinatal and infant health outcomes for high-need, low income, medicaid eligible individuals and their families.program serves 150 clients annually and 750 clients by the end of the 5- year grant period. To date, we have impacted well over 400 constituents and this # does not include the secondary audience of birthing individualsthat we have worked with re: their engagement in our childbirth through our home visiting services, we seek to improve the health and well-being of individuals of reproductive age and their families with a focus on individuals in the prenatal, postpartum, and interoception periods.chrf/lhvpn's pichc team connect clients to the following services: health insurance, doula and breastfeeding services, wic, food pantries, clothing and housing programs.targeted zip codes: mt. Black maternal child health initiative(january 1, 2023- december 31, 2025-funded by westchester county doh)primary goal: to improve birth outcomes in westchester county for black women in particular.
