The mission of the coalition of community health clinics is to improve healthcare for vulnerable populations residing in the portland-metro area. We strengthen community clinics, promote health equity, and foster collaboration across the safety-net delivery system.

Community health worker (chws) training - cchc is one of the states authorize agency to provide community health work training to community members. The goal of the training is to provide workers with the essential skills to support and empower underserved and under-resourced community members with health and wellness, health-related social needs, and health navigation. The training helps increase the pathway to healthcare by connecting newly certified chws to jobs with community-based organizations and community health clinics.

Expenses: $209K

Direct services cchc's community health workers provide health enrollment, health services navigation, and health-related social services and referrals to community members and clinic patients. Staff are part of a multi-agency mobile permanent supportive housing team that provides retention services for newly housed clients. The program supports member clinics to address social services and housing barriers and works with providers to connect community members to clinic services and supports
