DOCTORS CARE SEEKS FUNDING TO SUPPORT HEALTH NAVIGATION SERVICES THAT ADDRESS THE SOCIAL DETERMINANTS OF HEALTH FOR LOW-INCOME INDIVIDUALS. THE HEALTH NAVIGATION TEAM WILL IMPLEMENT IMPROVEMENTS TO INCREASE THE NUMBER OF INDIVIDUALS IMPACTED AND ADDRESS THE EFFECTIVENESS OF THE PROGRAM TO REDUCE AND SOCIOECONOMIC BARRIERS TO CARE AND IMPROVE OVERALL HEALTH.
