Hcpc worked with key local clinical, community, and faith-based partners to enhance the local public health infrastructure, provide outreach, education and connection to resources. Including but not limited to, access to health insurance, mental health support and aging services. In achieving program goals of:1. Access to health insurance coverage through partners with direct community reach and provide clear consistent messaging2. Build local public health capacity with partner cbo and fbo's through the development of non-clinical public health partners program.

Faith in prevention unifying communities, improving health outcomes for greater passaic county area and assisting new jersey in advancing important state health priorities. Recruit faith communities: hcpc will leverage its partnerships with a broad cross-section of organizations to identify potential faith communities for program implementation. This strategy includes establishing relationships and sharing information about faith in prevention (fip) with hcpc community action board (cab) member organizations, local faith-based (fbo's) and community based organizations (cbo) in passaic county to assist with recruitment efforts.
