The arizona supportive care (azsc) program is a medical management model that offers coordination of care to late-stage chronically ill patients struggling with daily living and disease management. The azsc team is comprised of physicians, nurses, social workers, and home care aides who provide home visits and who work in coordination with patients' community physicians. Additional services include disease medication, caregiver education, hospital-to-home transition support, symptom management consultation and 24/7 phone support. The outreach program provides psycho-social in-home services to patients and the families of patients with chronic illness who are not yet receiving hospice services. The supportive care for dementia program assists families in navigating the challenges of the dementia journey and shares its expertise with health professionals and first responders.

Expenses: $6.9M

Hospice of the valley offers primary care options for geriatric and underserved populations. Geriatric solutions is a house-call physician service practice for elderly and homebound patients with serious, chronic, and life-limiting illnesses but who are not ready for end-of-life care. Physicians and nurse practitioners provide comprehensive, personal primary care in the home and coordinate with specialists. Your health connection is an in-home mobile practice bringing medical services to home-limited patients in the comfort and convenience of their own homes, wherever they live. Our skilled care teams are honored to serve vulnerable children andadult patients who cannot easily access healthcare services.
