Building a healthy community by increasing access to and providing primary care, mental health, and case management services to populations with limited access to care.

Programs

Community health action - collection of public health programs that complement our clinical services to engage, educate, and support patients and community members to improve their health. We have programs to help people who live with chronic diseases like nutrition education, support groups for survivors of domestic violence, and regularly test for infectious diseases.

Mental health department - the mental health department has three main programs. The first provides group, one-on-one, and family-based therapy. The second are mental health programs based in local schools to teach students about health, wellness, and preventative care. The third is an addiction program that helps people in recovery.

Navigation helps clients prepare for office visits, to complete intake forms, attend appointments, receive treatment instructions, and arrange follow-up visits. Our model combines personalized attention, community knowledge, and linkages with a professional language services operation. Any dc resident with managed care can use our medical interpreters' services.

Program integration and evaluation - the program integration strategy aims to improve communication between primary care providers, mental health and substance use providers, community health workers, and language access staff to improve care coordination across programs and services and evaluate the success of our model.
