Integrated information technology:ncha manages a shared and integrated it platform for partner members who wish to use the platform. Ncha employs a director of information technology to oversee all program activities and has a services agreement with a hipaa-compliant external hosting company for hardware including database and citrix servers, system design and maintenance, and engineering support.in addition to basic e-mail, office productivity software, general ledger, and payroll programs, ncha hosts a shared practice management and electronic health record solution (athena practice management solution) that supports the integrated health care programs operated by alliance partner organizations. (continued on sch o)operating costs are shared on a pro-rated basis determined by number of organizational users.
Weld county:in collaboration with the weld county sheriffs office, ncha co-located care coordination staff within the jail to provide community-based re-entry planning services. Staff worked directly with incarcerated individuals to assess needs, develop transition plans, and connect participants to treatment, housing, and peer counseling resources prior to release. Services were available on-site during regular business hours, with coordination extending into the community to support continuity of care. The sheriffs office required monthly reporting on service activities and outcomes to track progress and inform ongoing collaboration.
0Other program accomplishments: in collaboration with the 11 county region 4 opioid abatement council, ncha supported increased access to treatment and recovery services for individuals by providing transportation and reducing other barriers to care.
