(continued on sch o)operating costs are shared on a pro-rated basis determined by number of organizational users.

Expenses: $2.3M

Weld county:in collaboration with the weld county sheriffs office, ncha co-located care coordination staff within the jail to provide community-based re-entry planning services. Staff worked directly with incarcerated individuals to assess needs, develop transition plans, and connect participants to treatment, housing, and peer counseling resources prior to release. Services were available on-site during regular business hours, with coordination extending into the community to support continuity of care. The sheriffs office required monthly reporting on service activities and outcomes to track progress and inform ongoing collaboration.

Other program accomplishments: in collaboration with the 11 county region 4 opioid abatement council, ncha supported increased access to treatment and recovery services for individuals by providing transportation and reducing other barriers to care.
