The organization's mission is to improve the quality of life for all northwest colorado residents by providing comprehensive health resources and community wellness.

Home services: northwest colorado health's home services department provides home health and hospice services across moffat and routt counties. These services provide skilled, compassionate and personalized care in the comfort and security of familiar surroundings on a sliding fee scale. Every year, the agency provides thousands of dollars of charity care to uninsured patient who cannot afford to pay and is often unable to collect payments for services provided to insured individuals (often times under-insured). The rural nature of our community continues to drive the need for care in the home. In 2024, northwest colorado health's home health staff provided 402 episodic care periods to home-bound clients throughout northwest colorado. Hospice and palliative care provide compassionate, quality care and enables terminally-ill patients to approach the end of life with dignity and comfort. In 2024, northwest colorado health hospice staff provided 2,460 hospice days across northwest colorado and traveled over 250,000 miles across the region to provide these services.

Prevention services: northwest colorado health's wide range of prevention programs reach community members of all ages and income levels and include wic, youth resiliency, health screenings, health education and many more. In 2024, we provided 357 cardiovascular screenings, conducted 1,422 follow up sessions with cardiovascular program participants, hosted 150 seniors in aging well classes and senior wellness clinics, and reached an average of 486 families per month through the wic program.

The organization also provides a residential assisted living facility with limited health care services for up to 20 senior citizens. This also includes community outreach in the form wellness, prevention and public engagement in aging well activities. The facility provided meals to seniors and had a total of approximately 7,100 resident days, a 97% occupancy rate.

In 2024 the organization added casey's pond senior living to its operations. This facility provides independent living, assisted living and skilled nursing services. The skilled nursing facility provides vital services to medicaid, medicare and priviate pay patients that would have no longer been available to the residents of northwest colorado due to receivership proceedings. Through the purchase of this facility, the organization was able to stabilize a declining population due to imminent closure, and thus residents were not forced to look elsewhere for these services. Due to our rural location, residents would have needed to move many hours away for these services, often far from their families and other support systems. The skilled nursing facility has 66 licensed beds, usually 60%-70% occupied by medicaid patients. The assisted living facility has 50 licensed suites, with the remaining 22 apartments for independent living residents. Total occupancy rebounded to 83% occupany by the end of 2024 after falling to 68% during the receivership.
