Our Home Health program provides services in southeastern Colorado rural areas where there are limited healthcare resources. We provide compassionate clinical care and multiple therapy disciplines in order to help each patient reach their goals and have successful rehabilitation outcomes.

The Sangre de Cristo Joni Fair Hospice House located in Pueblo is the only one of its kind in southern Colorado. Our staff is a professionally trained team of physicians, nurses, certified nurse assistants, social workers, chaplains, and volunteers who help provide quality and compassionate care in a home-like setting. The ultimate goal of our inpatient unit is to service as a place of transition, to stabilize patients and allow them to return home or to a long-term care environment. Our care focuses on the comfort of patients and their families.
