Our community mental health services provide personalized support to help individuals improve housing stability, strengthen social connections, and enhance mental and physical health. We assist people in accessing medical, housing, vocational, financial, and educational resources tailored to their goals. In 2024, our team worked alongside 1,417 individuals, offering case management, care coordination, adult rehabilitative mental health services (armhs), and behavioral health home services through touchstone connect in hennepin and dakota counties.
At touchstone, we believe stable housing is the foundation for well-being. Our community housing services empower individuals with mental illness to find and maintain safe, supportive homes. We offer two models to meet individual needs: - housing innovation & intentional communities these programs help individuals move into community apartments or homes with personalized support. Our dedicated staff provide mental health services, housing stabilization, tenant education, and connections to essential community resourcesright in the person's home. In 2024, housing innovation supported 127 people, with 77% maintaining stable housing for at least 12 months. Intentional communities served 59 households, and 95% achieved 12 months of stable housing. - supported housing we provide 24/7 onsite services at rising cedar apartments, new hope apartments, and minnehaha commons, ensuring residents receive the care they need to live independently. Services include medication management, mental health support, daily living assistance, housekeeping, emergency response, and care coordination. In 2024, our permanent supportive housing programs served 113 individuals, giving them a place to heal, grow, and thrive.
