Unified community connections, inc.'s mission is to support individuals with disabilities in achieving independence, productivity, and the quality of life they seek.

Programs

3 programs

Adult day programs offer men and women with developmental disabilities, 21 years of age and older, the opportunity to engage in meaningful individual and group activities. The organization operates four adult day program centers throughout the state. Day habilitation services focus on development of interpersonal, social and vocational skills are provided for other individuals. Adult day programs provided services to approximately 88 individuals with a wide array of disabilities and chronic health issues.

In-home supports assist individuals who need or want supports such as personal care, housekeeping and case management in order to remain in their own homes. Support services may be requested by the individual or a family member. Supported employment services provide assistance with developing vocational skills, job selection, interviewing, training and maintaining paid and volunteer positions in the community. Individuals are also assisted with finding and accessing transportation to and from their places of employment. No boundaries assistive technology center (the center) offers augmentative communication assessments and personalized training for children and adults with communication difficulties. Assistive devices are customized by staff to meet the unique needs of each individual. The center also serves as an equipment lending library.

The lift-equipped buses also provide transportation for day program participants on community outings and transport individuals in the residential program to and from medical appointments and recreational and leisure activities. Individuals are also assisted with finding and accessing transportation to and from their places of employment.
