Certified home health care & long term home health carelong term home health careparker's long term home health care program combines the comforts of home with the expert and experienced care of parker's skilled nursing facility. The program has been in existence since 1985.certified home health care certified home health care services are intermittent skilled care, provided under the direction of md orders. Home health care services are coordinated with interdisciplinary team members to ensure the most efficient plan for the patients' safe transition and stay in the community.participants in this program receive individualized nursing, medical and rehabilitation services that allow them to maintain maximum independence and remain in the comfort of their homes.our certified home health agency is licensed in brooklyn, bronx, queens, nassau, suffolk and westchester counties. Referrals may be made from an acute care or long term care facility, community, or directly from home.home care services include:-comprehensive assessment by a registered nurse-individualized nursing care plans-physical, occupational and speech therapy-social work and nutrition-home health aides-discharge planning-specialized services are available such as infusion and wound care-24-hour telephone availabilitythe certified home health care program provided services to 307 patients in 2024.

We are an intimate and unique program that provides personalized care to ensure your comfort, enhance your quality of life, preserve your dignity, and respect your choices.we are here for you 24-hours-a-day, 7-days-a-weekas an illness progresses, it can be difficult to make choices about care or know what to expect. Our compassionate team can help you explore your options. We respect your right to choose which services are best, as well as when and where you receive care.working with you, your care team will partner with your caregivers and personal doctor to develop a care plan tailored to your needs and goals.your services may include:-medical care, including all medication, supplies, and equipment necessary to manage your symptoms and relieve physical discomfort-guidance on what to expect as your condition changes-counseling for emotional and spiritual support-help with practical issues, such as advance care planning, medical decision-making, and preparing for the future.you'll have 24/7 access to care and services. We're here to support you however and whenever you need, including connecting you to services that the parker jewish institute has long been a leader in providing.the hospice program provided 9,378 days of care in 2024 for 106 patients and their families.parker jewish institute for health care and rehabilitation serves as an investor and owner in a collaborative limited liability partnership, agewell new york llc through its disregarded entity, parkercare new york, llc, a single member limited liability company. Agewell new york llc operates a new york state medicaid managed long term care plan ("mltcp"), medicare advantage plan and a medicaid advantage plus plan, which promotes managed long term health care for a broad section of the community on a charitable basis. Agewell sold the mltc plan, effective october 1, 2022, to another managed care organization. As of december 31, 2023 agewell new york llc has shut down operations.
